I'd go with the cave I suppose, would seem like the safer and more approriate place to live on account of how it protects against extreme weather and would allow you to actually get settled as long as there are no dangerous creatures inside.

Even though I've never had one, the idea of a toothache sounds much worse to me than a headache. I actually have mild headaches frequently, so while I don't think having one perpetually would be pleasant in any way, I think I could deal with it better than the other.
