I think most forum edits should cost Viz so maybe you can find out who did it through the viz/CP logs? I remember when Barathemos was doing forum edits in our forum, I was able to see everything in the logs.

Make sure the thing in the red circle is enabled. Everything else should also be enabled by default. I think that should fix it... I tried enabling/disabling it on my profile and it seems to disable/enable those missing boxes.
