Good question!

Normally, If it is chocolate milk then I will shake it for a good minute or two just to be sure that it is all evened out. Whenever I rarely get a cafeteria lunch from school which I try to avoid, I shake the carton a few times just for good measure. 

But if it is just plain milk or low fat milk then I do not shake it at all. Only when  it is chocolate or something else is mixed along with it.  Good question by the way!

I always forgot to shake my beverages. Always.

Regular milk? No, there is no reason to. Chocolate milk you need to shake though. The chocolate syrup is more dense than the milk and it will all settle at the bottom unless you shake or stir it.
