Well there was one time i was playing MW3 with all my buds on the map Seatown. Basically, we were pwning the other team pretty badly but they were still close to our score. Well, i pulled out my rpg and got ready to give the winning blow when all my friends were together behind me. So i shot the rpg... and it explodes in our faces, killing us all! The other team won because a sniper shot the rocket right when it came out! Right then i nearly chucked the controller at the screen in rage.

I don't know if this counts, but in the Spanish translation of Final Fantasy 7, the translators didn't translate Tifa's gender correctly, so only in the Spanish translation, Tifa has been mistaken for a transvestite.

Let me tell you about a time I got super super mad....
I was playing Metroid Prime: Hunters... I was up against the second boss of the game, the "Slench"
When he fires little green energy bursts, I had no Idea you could shoot them for energy pickups
I also had no Idea that the default Beam was super ineffective, but instead the Battlehammer was the best choice of weapon...
I was basically killing him bad, it was taking forever, And I got him down to almost dead.
Although I had about 30 energy left.
SO He charged me, and I died. I almost ragequit then, but I decided to fight him again.
And then I respawned. Back at the first world. 
I had forgotten to save.
It was then that I threw my DS across the room, and didn't play it again for about a month.
The "L" button on my DS no longer works =P

I'll just mention a fail that happened tonight... I was playing geometry wars retro evolved 2 on the ''kings'' setting and decided to try and get one of the achievements. This entailed activating 30 enemy zones without actually eliminating any of them. So, as you can imagine, the screen was jammed pack and nearly impossible to maneuver through. Well anyway, about 5 attempts in a row I got 29 zones activated but somehow could not find my way to the last one, lol. Oh well...

In Ocarina of Time, I was in the spirit temple. I tried killing some eye creature. I was with one heart left. I accidentally hit the moving spikes and I fell into my doom. It's basically dying castlevania style.

Trying to speedrun Ocarina of Time and getting all the glitches timed correctly...

In Pokémon Gold, I was going to face Lance, but was going to do it later. I turned off the GBA I was playing it on, then forgot that I didn't save it throughout the E4. Now here's the good part. When I finally got back to Lance, when he sent out his final Dratonite, I accidentally nudged the game cartridge, and the game froze. The worst part was, I forgot to save it again!

BF3; I was playing with my friend and spent 4 500% extended matches on conquest  vehicle maps and each and every time I almost got the 100 star colonel dog tags from this one dude, something stupid would happen,  e.g. random frag, misfired sniper head shot, a team kill, tank, a "god guided that rpg" moment, baited, and the match ends halfway through the shank. They are ALL mid-shank. Then the dude quit.
