The possible scenarios in my opinion:

1. The mafia accidently kills off the Forger without knowing during the night round.
2. Frod got forged because he was obviously one of the biggest targets and it JUST happened that Frod died from the hands of mafia (or 3rd party killer), so more power to the forger, I guess.
3. A loose cannon fired off.
4. A certain gun law might need to be looked at before more kills.
5. There's an assassin loose.

Plus, with experience as a Doctor in previous rounds, I would want to say this: It's highly unlikely to save someone on the very first night as the odds are against you. Otherwise, let's clap for the Doctor as he/she was lucky.
