I haven't been around for that long, but either Tournament Pool for Wii, or Chicken Shoot for Wii or DS. I have played some bad GBA games too, especially the later ones.

I have a huge interest in dinosaurs, so I tried playing a game called "Dinosaurs'' for the GBA, I think it was. The game was AWFUL.  I actually wrote a review for it, just because it was that bad.  Not only were the graphics terrible, but the game was totally boring and was completely confusing.

It would have to be Jade Empire for the Xbox. Don't get me wrong it was a good game with a good story, good graphics, and good controls; it just wasn't the game I expected it to be.

Although "Willow" (not the NES game, not the arcade game, the obscure PC game) comes somewhere close.

EDIT: Another recent thread has opened up the old wound. I'd like to change my answer. As I said there, it's "Ghostbusters" for the NES. I only began to list the problems with it there. The Angry Video Game Nerd, in his twenty-minute review, didn't even list all the problems. It is hard to imagine how human beings are capable of such incompetency--although I have heard that the game was rushed out in a matter of weeks.
