The anime Clannad, everytime Nagisa got sick I would be so worried about her, then she was fine, she fell in love with Tomoya, got married and became pregnant. She died giving birth to Ushio. I got to say the saddest anime death for me was when train saw saya die when he was just realizing his feelings its one of the few that have made me cry the only sadder one I can think of is the death in clannad after story though I wont spoil it for any who want to watch it.

"Spoiler Alert" I think the saddest anime death of all time would have to be Fire Fist Ace from One Piece. Everyone just fought so hard to try to save him in this massive battle for nothing. Plus One Piece doesn't tend to do a lot of death of characters, so it caught me off guard a bit.
