A lot of losses have to do with my stupidity. In Sword of Mana I tried to take down some enemies with the ability to sometimes reflect damage back without healing myself first. Needless to say, I was taken down. Several times.

Personally every final boss for me includes stupidity during the first run. The final boss is not supposed to be easy, and being stupid and experimenting usually allows me to find a way to beat it.
