Do you mean the title on the right side of the post?

To do that you have to go to the "Custom title" section under edit profile and settings.

Into the section that says "Custom Title" you'll have to use the image tags and put the URL in the middle as such:

[ i m g ]   URL    [ / i m g ]

(Without all of the spaces, I had to do those to get them to show up)

Barathemos is correct. However, make sure that the URL you use is short enough. If the URL is too long, part of it will be cut off. 

Try uploading your picture to imgur.com

Make sure the URL looks something like http://i.imgur.com/yourimg.gif

It should be a direct link to the picture.
