The dead levels are typical of Arcades is when a game cannot continue supporting your game play
that means a unexpected game over , the first time when a dead screen was discover was in PacMan
after reach the level 256 this happens




other classic game with a dead screen is the Donkey Kong for arcade, in the level 22 the time is too short and
you got a game over after 3 or 5 steps. In Zelda for NES if you die more than 100 times all the save data
will be erased and in Pokémon exist Missigno a bug Pokémon that erase the save data if you capture it

You missed the one in Dig Dug where an enemy would stay exactly where you spawn-
Slowly draining your hard earned lives.

A dead screen I know is from Duck Hunt, reaching level 100 will count as "R=0". Once the level starts, the Ducks are just everywhere, sometimes the rounds will skip making it impossible to win. Also annoying laugh from the dog. woop-di-do.
