Would it be a good idea to have Mednafen (in full screen mode) have a fixed 4:3 aspect, I like fullscreen, but not when I have to dart my eyes back and forth to see everything.

If there are no settings in the emulator that you can change for personal preference, then I doubt it could be done. The emulators featured in the plug-in are mostly those that have been individually programmed by other developers, and likely have copyright issues attached to editing them. Not only that, but the plug-in seems to have direct draw errors when you change the resolution. I honestly doubt it.
