I would have to say any monster from Dead Space. I am a big ol' chicken though, so of course they scare me. Yet, I would also have said Amnesia: The Dark Descent if it hadn't been for someone else already claiming that one. I played Amnesia and it was the one game I nearly had to stop because I felt like it would be a heart attack. The monster bashing down doors and stuff and chasing you without a way to kill him... Ugh >.< Also, the water monster in Amnesia was kinda scary. Whenever you saw water you always knew it was going to chase you and it would only take a few hits before you died. So... Maybe exciting and not necessarily scary.

Don't forget the Nemesis from Resident Evil 3, He stalks you for most of the game.
