When I try to burn an iso image to my DVD+R with Poweriso, it says the disc is not empty.
I checked, and the disc is most certainly empty! I even formatted it to be sure.
I don't know what is causing this problem, and would like some help on how to fix it please.

There has to be something on the disc, that's the only explanation.

Also you cannot format a DVD+R, once files are on it, they are on it. The only rewritable discs are DVD+RW.

You're more than likely gonna have to get a new disc, or you could use Daemon tools, Very very useful virtual disc tool.
