I don't know about worst, but my all-time favorite hack would have to be Sonic Megamix. It's an amazing hack of Sonic 1 for the Sega CD (though previous versions of the hack are on the Genesis) that has a lot of great levels, abilities, characters and other stuff. It way better than the 2D games Sega makes nowadays

Anyway v3.0 of the hack is in the RGR, so I suggest you try it out if you're into Sonic games!

My 3 favorites are Pokémon Rijon Adventures, Pokémon Ruby Destiny: Reign of Legends, and Super Mario Bros. Crossover. SMB Crossover is actually on a different site, but it's a great game. It's the original SMB with a bunch of other characters like Samus, Mega Man, and Link.
