JigSaw is finishing up a new system for custom layouts. The last post I saw from JigSaw said it would be another 1-2 weeks before he's done. I think the users who have custom layouts now are either staff or had them before they stopped the layout system. Someone had an inappropriate layout, so they shut down the whole system until JigSaw finishes the new one.

David has been really busy lately, so you probably won't get much help that way. My best advice is to just wait it out. I have two premade layouts now that I'm waiting to use.

Currently there is no way to get a custom layout submitted, it seems that will change soon, so keep an eye out.

Only Vizzed Elite and above can set their own layouts at the moment. I imagine there will be some official submit form once Jigsaw gets the layout maker done.
