Probably Elfen Leid. I haven't actually seen/read it, but my fiance has and he's told me a few things about it. From the content I would have to say that it is the most controversial one I've ever heard of. Here's a link to Wiki where you can read about it.

The second I read the title I thought Elfen Lied. I've watched it quite a few times and it's definitely not for children...or the elderly. It has a lot of gruesome, bloody deaths and some of the characters are fairly young.
