mine would most definitely be dragonball z budakai 2 (gamecube). i know i know. some of you are laughing. it's a really good game, but for me it was just super difficult. especially fighting kid buu. i played that game for well over 80 hours before i beat kid buu. man, talk about a major head ache.

007 the World is Not Enough for the Nintendo 64 was the hardest game I ever played. Especially the submarine level. That was killer. I didn't think I was ever going to figure out how to get out of that place.

battletoads... can't beat it one player, and also, hitting eachother in 2 player.... nightmareish game.

but one game from my childhood that's always haunted me is subterrania for genesis. hardest ship flying shooting pick up soldiers to save them game ever. can't even land the ship without trying a few times and taking damage.
