well, for one. check you hero's speed. if its higher than the monsters by alot, thats the reason. speed basically goes both ways. kinda like comparison. whichever one is higher, that one goes b4 the other, and the hit rate is hitened, or lowered, depending on how you set it.
Sometimes, good isn't good enough... Even the slightest difference between good and great, can determine the fate of the world...

Another idea would be to check your hero's equipment. In RMXP, I was surprised at how much difference just a couple of points made... My hero was going through an area with practically no equipment and getting creamed by the monsters, then he got ONE HELMET and all the monsters did zero damage. So check the speed bonus on your hero's equipment.
