Several times, in fact.

While it happens occasionally, most of the times were when I first started.

Since I have the PC version, I just hit
F3 and the location coordinates appear. They're not hard to memorize (or you could save it somewhere).

It's also helpful to keep F3 running, because when you die, the screen will still retain your coordinates. This is very helpful in finding your lost stuff.

When I first started playing Minecraft and also when I returned to Minecraft, every time I died before I set my bed, when got killed my a mod in a cave, I respond like a biome away from where my house is and I don't remember which way to go, that was until I know where my house was when I had the HUD up by pressing F3, and I marked where my house was and thus, don't have any issues from that point on.

This has happened to me many times, and I hate it when it happens because it can be so hard to find your way back.  Most of the time it had involved me walking leaving my house, getting mobbed by a mob, dying, and spawning in a place where I have no idea where my house is. There was this one time where my house was maybe 20 blocks north of the spawn, and one day, I die and head in the wrong direction. I was wandering for about 45 minutes before I decided to commit suicide. Then guess what? I spawn, TURN around, and there it is, my giant, 12 story tower.

This has happened to me so many times it's not funny.

One of the more memorable instances was when I was a complete noob. I built countless nether portals and traveled through them all. When I finally landed back in the over world, I was in terrain so strange I thought I had glitched the game. Anyways,I built a house out of obsidian and tried to sleep in it. I couldn't, because the roof was too short. Later that day, I died and landed at spawn. I spent weeks looking for that house, and when I did, it was beautiful.

I used to when I first started, but after a while, I learned to stay in one place for the most part until I was pretty well entrenched in an area.  By the time I have a modest sized home along with everything I need to survive comfortably, its usually pretty easy to see from the map, making it fairly easy to find again once I start exploring.  In addition to that, I tend to set up some pretty good lighting both on my home and the surrounding area, making it easier to find during the evenings (Especially since that's the time you need shelter most desperately.)

This happened all the time before I found out how to pull up the x y and z coordinates. One of the times I did this I was coming back from a successful mining expedition, I had diamonds, but I couldn't remember how to find to the exit of the mine. Long story short I dug straight up and built a dirt house. After some exploring in the day I found a village, wasn't a complete waste of time I guess.

Chindogu :

By the time I get that deep, I tend to have a LOT of extra materials and will typically leave a trail of torches leading back to the surface (With redstone torches at the entrance of any dead-end/fully explored path.  When I go on a mining expedition, I will usually leave with the following: Stone Pick axe X 5, One Iron Pick axe (Diamond if I can comfortably spare the resources.) a stack of bread, a full stack of torches, an Iron Sword, and a stack of redstone torches if I plan to do some exploring.

If I feel particularly ambitious, I will bring a few doors down with me.  Then I will block off any dead-end paths and leave a door so I can go back if I want to.  I used to leave signs near the doors, but after a while, I decided it was more effort than it was worth.

I also know someone who simply seals off any path he inst using, simply leaving a single block out of the wall with a torch in the hole so he can find it again later.

Step 3 or 4 for me in any Minecraft world that isn't PVP is to build a map so I know where the heck I am.

If I don't have it then I'm just woefully lost. I'll get turned around so bad I'll probably never even see my house or materials again, and I'll be back to punching trees for wood.

Oh don't even get me started with this! I would always forget where my house was! I don't know why but I would always venture off of the main spawn point to build my house. And while making my house, BOOM . . . A creeper blows me up. So I lose my map and everything else and guess what!? Oh great it's already night time. So I'm just screwed right then and there. I would have no idea where to go and I would just keep dying. That is why I hardly play minecraft anymore xD .

I once forgot where I had my dirt house because I went off into a faraway jungle for wood, and I went too far, and I had to rebuild my house with wooden planks.
