It's really a mix for me. I mostly play games for a well written or interesting story and world but I'm also drawn to some purely for their visuals (looking at you Ookami, Terria, and any other game with unique graphics). The story is the main draw for me, but jarring animations or poorly sculpted/textured models and environment take away from the immersion in most cases. I do like when games have a good soundtrack but I mostly mute that anyways and listen to my own music. I like interesting characters, I need something more than "grrr I have mudda f'ing psychic powers yo and imma get revenge!" I like characters with a little more dimension than that. Ok, your character in Skyrim or any MMO (WOW? ROM?) literally has no backstory (or if it does it's very flimsy) but that's were the player comes in for games like that. Even if the backstory is explained through seemingly useless interactions or cut scenes I feel there should be some.

If I'm to be totally immersed in a game, I need three things; a good story with good characters (I feel those go hand in hand), addicting gameplay, and a good sense of atmosphere.
If the story's crap, I'll still play it and all, but I won't really care. I'll just be doing everything because I have to. There should be a sense of WANT and just HAVE TO in the games.
If the gameplay is really bad, I'll drop the game for a different one that won't make me go bald. Probably the biggest turn off a game can have is if the gameplay just doesn't work.
Atmosphere I suppose is a combination of sound and graphics. After all, if I go into a haunted cave and I don't get creeped out, what's the point? Without a fitting atmosphere I can't feel immersed in a game. I just can't.
