I remember back in the day playing Vice City, and San Andreas and trying to get all of those statues (or whatever they were). It's nearly impossible without spending Hundreds of hours, or having a guide to show you where they all are.

The Emeralds of Sonic 1... That labyrinth is pretty harder, touch one of those blocks that say "Up" and you will see. Same thing in the Master System/Game Gear version. They are very well hidden.

If you want a challenge, then beat on of the 2 N64 Banjo-Kazooie games 100%. Finding all 900 musical notes without a guide in the first one is... -*Expletive*- HARD. Those empty honeycombs were hard too... I actually found the last of those after I found all the jiggies AND all the notes.

The second game has a few insanely hard to find/acquire jiggies and spellbook pages too.

I never had the patience or motivation to get the Biggoron sword. Seems like a lot of work in OoT just to get a better sword.

I wouldnt say hardest.. but getting all of the health upgrades and weapon upgrades in Super Metroid wasnt easy.. especially without a map.  A worthy mention would be the hidden boxes in GTA 3.

Hardest one I think to get was the super missle in metroid zero mission it was in the first main area where you have to have the speed running and have ball form. I still can't get to it though.

I feel that there is a really strong weapon in cave story(not the machine gun, nor the snake, but before you do the messed up egg corridor) I think I might get a really strong weapon if I go to first cave and talk to the guy that you steal the polar star from. It's really hard because I keep on dieing when fighting Omega, he jumps on me and it nearly kills me instantly. I got past him ONCE but just to die on the flying dragons and I only had the snake with me that time. It's so hard!

I'd personally say all of the Sun and Moon medals in Sonic Unleashed are the most difficult for me, mainly since I don't know many other games I've tried collecting things in like that. Not only do they unlock more levels for you, but they are hidden all over the place, Nya~. Finding them would take forever, since they're not as easy to find as the Red Stars in Generations.

That said, second most difficult would be Sonic Colors' Red Rings. I think I've only found 10 of them, and I've had that game since release .

As I have not been playing too many video games recently, and I doubt that I can muster enough memory and brain power at the moment to recount a few of the long and hard hours that I have spent for individual items, I will discuss just a few that I can remember off of the top of my head.

When I first started Battle Front 2 for the PS2, I found getting speeder bikes and driving them to be extremely difficult. That was even the case in Yavin 4 in which there are 2 relatively easy-to-get bikes on both ends of the map. Usually what would happen is a teammate would take the speeder and drive off with it, so I could not use the one on my side and the one on the other side had too many enemies around it to get it without dying. The sandbox skull in Halo 3! Dang it was hard and in a very hard to get to place and while you're getting it, the guardians were shooting at you with deadly laser rays! Finally me and my brother gave up and downloaded a map that allowed you to get there easily.
