For me, it would have to be Back to the Future part 2 and 3 for NES. I am not talking about the Vizzed version of the game that includes a 'save' option, I am talking about the original version itself. In order to get anywhere on it, one has to draw a map and even then it is still quite difficult and time consuming. I remember so many times, I would get quite far only to die and have to start again.

For me, it would be Ghosts and Goblins for the NES. That game is just insanely difficult. It used to be Battletoads when I first got it for my NES when I was in kindergarten, but after a few years, I could beat that game 3 out of 4 times.

Have you ever played I Want To Be the Guy? It's free for the PC. It's pretty much the most unfair, while not being completely unbeatable, platformer ever made. It's like Super Mario Frustration, but not completely rigged.

I haven't played it in like ten years, but my father has a copy of Beyond the Beyond for PS1. That game pisses me off so bad. Way too many freaking 'random' encounters of enemies. I couldn't go two steps without hearing that stupid music. Granted, I usually ran away unless I was feeling particularly ambitious about leveling up - which wasn't often since the battle system got on my nerves - but still. It annoyed me when I was lost in all the maps and caves and I was just trying to find a way OUT, and I couldn't because the enemies didn't know how to leave me alone.

Mike Tyson's Punch Out. :3

Just kidding, but "Glover" really makes me want to punch something. It's repetitive, sounds awful, and is just plain boring. Reallyl frustrating.
