I do songs on Difficult/Expert for about half an hour a day on DDR Supernova and its sequel most of the time.

DDR=Dance Dance Revolution; game in which you step on arrows to follow arrow sequences on the screen

On a scale from sedentary to athlete, how active would you honestly consider me to be?

I need it to fill out a fitness survey.

30 minutes of moderate exercise a day is the minimum to consider yourself active.  Video games especially, since they are repetitive movements, aren't as helpful as doing something like walking, running, or playing sports or dedicated fitness classes and training.

I'd put you at average.  

You can probably look at yourself in the mirror and decide your level of fitness without much issue, too.

How many times do you do your activity. If you are doing it 3 or 4 times a week. That meets the criteria for moderate activity. If you are doing it every day or at least 6 times a week I would say active.

I, myself, have played Dance Dance Revolution on Easy (Level Two) - even THAT is hard! And I only do it for about ten minutes, and not everyday either! But doing the Difficult/Expert version, for a whole half-hour, and everyday too is definitely a good sign that you are active. Keep it up!

That is pretty active. It would be nothing a few hundred years ago, but now that is really fit. DDR is really active depending on what mode you play on, and it sounds like you do it right.

Its better than sitting idle all day but I wouldnt say its above average. You're health is unlikely to be in any kind of danger at any rate.
If you want to judge your health take a half hour jog round a park and see how you fare with that. If you manage it without feeling too tired you're in pretty good condition.
That's assuming wherever you live has green space.

Yeah, I'd say at best, you'd be in average physical condition. DDR is a video game, albeit one that has you move a bit. But I've played DDR before, and never felt like it was a bigger physical challenge than running.

If your goal is to be active enough to run the mile in gym without trouble, then it's great, and you'll probably be able to do so.

However, if your goal is to boost your cardio health to a level where you could run a 5k without difficulty, then I'd have to advise more running, and less DDR.

I wouldn't say you are "active", but you are doing something, which is better than nothing.

I think you're fairly active. You get exercise, but do you get outside enough? You probably do, the minimum is a quarter per day, something I already do when I cycle to school. If you keep this up, you don't have to worry about your weight and/or health.
