I have to say the game that made me jump the most is RE3. You be finding gun and notes, then a monster would bust threw the wall yelling STARS  I jump out of my skin  every time. But it was so much fun.

I'd have to go with Dead Space (1 and 2). I love how when your HP runs out the monster that killed you takes its time to rip you apart before the screen fades out. And they're both so good for "jump" scares it's ridiculous. It's like Resident Evil 4 in space with scariness pumped up times 100 (and no babysitting a helpless girl one third of the game )

I don't play too many modern games, but I would have to say that as a kid the ending in Shadowgate was kind of creepy.  I think the combination of the eery music and the picture of the grim reaper went pretty well together.  What added to the scary factor for me too was that I was 4 at the time I played that game.  Then again I remember that whole game being kind of creepy in general.

Then, it spawns you to the nearest check point. As a kid, I was horrified because of the lighting and the amount of detail that was put into it. Gave my nightmares, actually.

While it's not a game over screen, one of the most terrifying parts of a game was when I was a real young kid, me and my family, plus some cousins and aunt and uncles went to an arcade about 25 minutes from where I live, and unfortunately the arcade is no longer around, but the game Sinistar, had the Sinistar character going around saying "BEWARE I LIVE" and doing that roaring noise, and that scared the hell out of me as a kid.

Now when it comes to game over screens, I would have to say Ninja Gaiden in the arcade, as a kid this made me feel a bit scared. There was this local mini mart that had arcades, and Ninja Gaiden was one of them, I mostly watched my dad play the game. Here is the game over screen below

For me, it's going to be the endings to the Five Nights at Freddys games, you know, when evil animals just jump out at you and try to eat your face and all. I don't normally like playing games that can even lead up to a scary ending, just never had a lot of experience with them, bur I saw a bunch of Lets Plays of on YouTube of people playing FNAF, and the endings get me every time. Because of all the stuff that's going on in the game, you're even scared If you win.

When you think you have done well and do what you need to do. oh...you can never be more wrong I was so scared for dead space and I was like yep a sequel is going to be made for this game!
