Mine would have to be of recent. The final boss of Fire Emblem Awakening on Lunatic + mode. An infinite number of enemies with thousands of possible abilities made for one of the toughest runs of any game I have ever played.

Metal Gear Solid 3: Snake Eater. I got stuck when I had to kill Fury (one of the 6 Cobra member's.) Normally it's pretty easy, but I decided to play using Hard mode.. I had so many close calls. Like Shoot him one more time and he will die, but he always got me.   Pretty annoying, but in some ways it was pretty fun.

I'd say the hardest final boss for me was Sigma's second phase from Mega Man X3. He's immune to every weapon except a charged mega buster, which only does a tiny bit of damage, or the z-saber, which is almost impossible to hit him with. He also takes up 75% of the screen and can take off half of your health in a flash. Good fun.

Hardest boss for me was the Sephiroth platinum match in Olympus Colosseum in Kingdom Hearts II. That fight took me literally days. I tried and failed easily over 200 times to beat that dude. 

For some reason my friends managed to beat him in like 15+ tries, but I just couldn't even contest with that massive sword wielding jerk.

I'm gonna go in a whole different direction, and say Isaac Frost in the game fight night champion.  Yes, it's a boxing game, but it has a great story mode and he is the final guy.  The whole story leads up to him, and it is a very difficult fight.

- There were also some pretty difficult bosses in World of Warcraft.  Like old school Yogg-Saron and C'Thun.  But these were 20/40 man raids.

Sepiroth in both Kingdom Hearts and Kingdom Hearts 2 (but primarily in the first) can be extremely hard especially if you're not prepared.
He is easier in the 2nd game then the 1st but I still think he's pretty hard. (Otherwise the 2nd game isn't too hard for the most of it).

I'd have to say Endless Discharge (nasty name I know) on Dark Souls without cheating. Fighting that jerk without using the glitch to your advantage is near impossible. He's not as bad as the Hell Kite though. (I haven't defeated it yet so I decided not to use it.)

For me, I'd have to go with The Shadow Queen from Paper Mario: The Thousand Year Door. That was such an easy game, it totally shocked me how hard this boss was. The first phase was OK, but the second phase was a forced failure phase, so naturally, I wasted all my items on that phase. Then the third phase comes along, and it's all like "Hey, your dead!" Then I had to watch the 5 day long cut scene that comes before it AGAIN!!! The only reason Bonetail isn't here is I haven't gotten to him yet.
