First you need to become a normal member by making 10 posts and after 7 days of registration.

Then after that you have to have a good reputation on the forum by having good quality posts and posting often. It also helps to be friendly and patient with dealing with all members. If people think you are a good member for the forum they will give you Trust Points (TP) and you need 10 TP to be trusted. Be warned that if you get someone upset with you or bump into the wrong person at the wrong time they can take TP away from you without you know who took it away.

You are told who gives you TP but you are not told who takes away your TP.

The ranking up thing is just a cool little feature that's affected by your post count. The more posts/threads you make the more "experience" you acquire and thus you go up in level. The feature itself has no affect on anything that goes on with the board, however. But it is useful to intimidate newbies.

I think Cyro has your first question covered. To answer the second one, no you cannot earn TP without someone giving it to you. The point of the system is to be recognized by others, whether that's another trusted member, a mod, or even David. Be more active and get to know them if you wish to earn some TP.

A slightly more in-depth version of what rcarter2 is that by posting more (which may help you go up in rank) will increase your level and then increase your RPG stats which combined with items will help you in the arena feature which might not be finished by sometime next year ( but let's hope not).

To add on to what everybody else stated about the TP: It's not hard to get. I obtained 11 points within the course of two weeks (not even). The big thing is just being a decent member. Being extremely active gets you noticed. Once you've been noticed? Having quality posts will do the rest.
