Mine was at the end of Borderlands. I was on my way to the vault, expecting to find some awesome weapons and stuff, but ended up fighting a tentacle monster that lasted 30 minutes and gave crap rewards. Not 1 single alien weapon among them.

Mine probably would have to be when I was playing Kingdom Hearts 2 and I came across some cool looking dude (only to later find out about his epicness) named Sephiroth. The first thing I noticed was his health bar! Like omg, and then after doging his first blows, getting some hits on him, then next thing I know I was 2HitKOed!!

It wasn't TOO surprising when it happened, I kind of expected it really. What happens after is a mission called Aftermath, where you crawl out of the helicopter you've crashed in. You get to see the destruction of the blast first hand before you die from who knows what injuries you've sustained.
