Metroid 1 (bad game IMO) had no map system, little instructions and cryptic puzzles with no hints, great game right? But really, if someone released a hack with a map and puzzle hints, it could actually be a great game IMO.

When I'm so good at the game that I become underleveled. Luckily, I enjoy doing some grinding every now and then. Final Fantasy makes me do that constantly, and it can leave someone very confused on how to beat something at first.

Nothing in FF will ever be more confusing than the Ice Fields of FFVII's Disc 2, where you have the most vague map you could imagine, huge areas with no landmarks, powerful enemies, and the constant fear of dying from hypothermia. it's not a very pleasant point of the game, clearly.
It's a massive area that doesn't burst and flow forth with Lifestream, but with utter confusion as to where you are and how to get to this "Cave" from wherever it is you are.

The most annoying thing in an action game is when you are stuck at boss you can't beat and there is no way to get back.  In a strategy game it is when you have to make a choice over who has to die and there is no way to save all of your party members.  This is really annoying in games like Fire Emblem that make it so you can't use your party members any more if they are defeated once.  In puzzle games it is annoying when you think that have solved the puzzle but the game has decided that the way you think it should solved, is not how it is going to solved.  In shooters, the most annoying thing would have to be the enemies that never die.  Like the un kill able necromorph in Dead Space 2.
