Yep, I've failed 3 or 4 times, but it's because I forgot the : or typo'd their name, and if you edit your post, it doesn't summon them.

The first summon I did was in my birthday thread on July 12th of
this year. I didn't know how to summon so I spammed my own thread ( I
didn't mean to though ) . One of the people in that thread showed me how .
I tries to summon legacyme3 but failed because I didn't put the 3 at the end
of the user name. Leggy told me this and I felt kind of embarrassed . People
make mistakes   Now when I summon people I don't get it wrong that much
anymore.    lol

I have only done one successful summon, I never get it right or it doesn't work for me. I have the worst luck when it comes to summoning people, because when it doesn't work I look like an idiot lol.

No I don't believe I have yet. When I summon I try to remember to check I spelt the name right because I discovered there is no way to summon users when you edit a post. :/
