I'm playing Pokemon White and I've caught more Pokemon with Pokeballs than Ultra Balls
at some points in the game a Pokeball will catch a full health Pokemon but when i use an Ultra Ball it always pops out right away
Anyone else having similar issues?

It is all up to chance, kyuremu. ultra balls increase the chance by 2X, but chance is chance and nothing can cotrol it.

but they've never failed this hard i couldn't even capture a Sewaddle at level 12

If you don't like battling a Pokemon to catch it, just buy a lot of Quick Balls; they have an astronomical Catch Rate on the first turn of the battle, but they are worthless after that. However, if you use them on the first turn, you can catch even level 60 wild Pokemon at full health. Just make sure you have plenty of Net, Dusk, and Ultra Balls in reserve just in case that Quick Ball fails, because the second one won't even have half a chance of catching even a level 15 Pokemon. Oh, and if you're playing at night, Dusk balls can catch Pokemon at full health relatively easily up through about level 45 or so.
